Once you register you will receive on order form to pre-sale tickets. $10 per ticket sold can be applied to your child's fees. PLAY FOR FREE (Opportunity starts March 2018 an ends May of 2018)

Football Registration Fees


(a $50 non-refundable deposit is needed to register.)
**ALL fees become NON-REFUNDABLE & NON-TRANSFERABLE after the 1st day of practice**

This fee includes football equipment (helmet, shoulder pads, game jersey, and a mouth guard). This equipment is the property of the Storm and must be returned at the end of the season. Parents will need to purchase game pants, cleats, socks, additional mouth guards and an athletic supporter.

Cheer Registration Fees

$225 plus cost of uniform

Why does cheer cost more than football? In Cheerleading you need a custom uniform tailored to your child’s measurements which will be theirs to keep. The Cheer director & the head coaches will set a date for fittings to be done. The uniform cannot be ordered until you are paid in full. Uniform must be paid in full on or before June 15 or your uniform will not be ordered for the season. There are no exceptions.

In-Person Registration Option

Storm Board Members and Coaches will be available to answer questions and sign up athletes starting on February 12,2018 at the Round Table Pizza located at 1400 W Campbell Ave, Campbell CA 95005 from 6:00 p.m. to 7:30 p.m.

Parents can choose to download and complete the Pal Registration Forms on this page and mail them to the address listed below along with a $50 deposit check made out to West San Jose Storm. Please note that (2) two additional forms will need to be filled out in person prior to the first day of practice.

2018 PAL Code of Ethics Form

2018 PAL Player & Parent Responsibility Form

All Athletes are required to provide the following items as part of the registration process:

  • Certified Birth Certificate – (cannot be a carbon copy) All new players/cheerleaders must submit an original certified copy of the players Birth Certificate
    (they will be returned to parents after certified by PAL)
  • it is the sole responsibility of the parent/guardian to collect the birth certificate back from Storm
  • Paperwork can be turned in at any registration date or turned in on the first day of practice.
  • Any participant who registers online, by fax, by mail or in person is still required to print/complete all forms requiring signature and make sure they are received by WSJ Storm by the first day of kickoff  July 22, 2018.
  • If mailing forms in they must be received by July 13, 2018
  • If walking forms in they must be recieved by first day of kickoff July 22, 2018
  • **Children will not be able to participate on the field if any forms are missing**



On Line Registration Options

Currently, Storm is in the process of setting up systems to collect the registration forms electronically.  Until this system is completed, we can only currently accept payments for registrations online at this time.  If you chose to pay for registration this way, please down load and complete the required registration forms and either mail or email them to our registration coordinator at mailto:shannon@wsjstorm.com
